Information collected
To provide the service, Cognisance may need to collect and maintain personal contact details and therapy session records. At initial contact, this may include your name and contact details so that appointments can be arranged or changes communicated.
During therapy, a personal details form may include your name, address, date of birth and GP contact information. Louise will not contact your GP without prior discussion unless there is a duty of care that requires it.
Lawful basis
Personal information is processed in accordance with applicable UK data protection law, including the UK GDPR and the Data Protection Act 2018. Special category information is handled as necessary for the provision of healthcare and therapy, and for the relevant contractual or legal purposes.
How information is used
- To administer, schedule and reschedule appointments.
- To communicate appointment times and therapy-related information as agreed.
- To maintain brief notes of sessions and track progress.
- To meet legal, professional, safeguarding and accounting obligations.
Confidentiality
Sessions are confidential. Confidentiality may only be broken where there is a legal, ethical or safeguarding obligation, such as concerns about abuse, neglect or serious harm to yourself or others. Where possible, Louise will discuss this with you first.
Retention and security
Personal information and therapy notes are stored securely and retained only for as long as necessary in line with professional and legal requirements. Therapy notes are retained for seven years after therapy ends.
Email accounts and devices used to communicate with clients are protected by passwords and security measures. Unnecessary email correspondence is deleted when it is no longer needed.
Sharing information
Limited personal information may be shared with carefully selected third parties where necessary for invoicing, booking or another specific purpose connected with the service. Information is not sold or shared for marketing or research without explicit consent.
Website visitors
This website provides telephone and email contact links. It does not use a contact form or analytics cookies.
Your rights
You may have rights to access, correct, delete or restrict processing of personal information. Some requests may be limited where information must be retained by law.
